CERTIFICATE OF INSURANCE REQUIREMENTS:
Minimum of a $100,000.00
Certificates must show Liability and Physical Damage:
- POLICY NUMBER
- EFFECTIVE DATE AND EXPIRATION
- LIMITS
- PHYSICAL DAMAGE AND LIABILITY DEDUCTIBLES
- TYPE OF COVERAGE
- MINIMUM OF 10 DAY CANCELLATION NOTICE
- SIGNATURE OR STAMPED SIGNATURE OF AUTHORIZED REPRESENTATIVE
The Certificate Holder must be Nation-Wide General Rental Center, INC. and all certificates must be worded as follows:
“Nation-Wide General Rental Center, INC. and its Partners are named as Additional Insured and Loss Payee for all equipment Leased to or Rented from Nation-Wide General Rental Center, INC. including Substitute, Extra, Permanent or Interim equipment.”
